Student Complaint Form
EOSC students may need to pursue questions or concerns involving academic or nonacademic aspects of student life. Please refer to the Student Handbook
before filling out this form. There are detailed instructions in the Student Handbook on the processes and procedures for student complaints.
A student complaint may involve an EOSC employee (faculty, staff or student –employee) or other students. Students with a complaint involving an employee should first seek to resolve the issue with the employee. If the matter cannot be resolved with the employee, the complaint can be forwarded to the employee’s supervisor. This form is to be used to submit a formal complaint when a student has been unable to satisfactorily resolve an issue with the faculty, staff, students or others involved.
Complaints may be submitted anonymously; however, unless you include
your contact information Eastern Oklahoma State College may be unable
to investigate your complaint and will not have the ability to respond back to you regarding an
Types of Complaints
Please provide detailed information and choose one of the following options based on the type of incident you are reporting:
– For general concerns or actions that are
experienced or observed involving a student, faculty or staff member; or
concerns that a student has violated the Student Code of Conduct.
– For incidents or violations involving violations of EOSC’s Academic Policies and Procedures.
Residence Hall Issue
– For incidents or concerns involving an on-campus residence hall or apartment.
Safety Concern or Threatening Behavior
– For concerns or actions that are experienced or observed and present a danger to an individual or the EOSC community. If this is an emergency, please contact Campus Police immediately at 918.465.1718.
Description of Incident or Concern
Please provide a detailed description of the incident/concern using specific
concise, objective language (who, what, where, when, why, and how).
Please also include any history of the incident (what steps have been taken previously to remedy this situation? What has been contacted and when?)
